What this document covers
The Lactation Treatment Consent describes what happens at a lactation visit: the scope of an IBCLC, CLS, or CLC visit; what to expect at an in-person, home, or telehealth appointment; the typical risks and benefits of common lactation interventions; alternatives to recommended care; how telehealth visits work; and our use of AI-supported documentation tools.
How it fits with the other forms
This consent is reviewed in addition to the four intake documents when you schedule the corresponding service. You only see and sign it when that service is on your appointment.
