    Payment responsibility, insurance billing, card-on-file, cancellation fees, home-visit fees, and refund terms.

  • Master Intake Acknowledgment

    Single-page acknowledgment that you have reviewed the four core intake documents.

New clients receive these documents through Jane, our practice-management system, when an appointment is scheduled. You may also review them here first and email questions to admin@bayoucitybreastfeeding.com before your visit.
