Curative is a regional health plan operating in select markets across Texas, Florida, Georgia, Maryland, and the Washington DC area, with a unique $0-deductible structure for members who complete a baseline visit. We are in-network. Most patients see no out-of-pocket cost for lactation visits. Verification confirms the specifics for your plan.

Curative plan exceptions

We are in-network with most Curative plans. There is one situation where we cannot bill Curative for your visit. If it applies to you, you can still book as a self-pay patient and we will give you the rate up front.

  • When the baby is the only one insured

    Curative does not cover lactation visits billed under the infant, and we cannot bill a visit on a baby’s policy. If your baby is the only person covered by Curative, the visit is self-pay unless a parent is also on a plan we can bill.

How it works

How we work with your insurance

  1. You submit the intake form

    Tell us about your plan and what is going on with feeding. The form is HIPAA compliant and routes directly to our billing team.

  2. We verify your benefits

    Our billing team contacts Curative to confirm in-network status, baseline-visit completion status, and any plan-specific limits.

  3. You get a written estimate

    Before your first visit you receive an email with verified benefits and the expected out-of-pocket cost. For most Curative members, this is $0.

  4. We file the claim

    After your visit we submit the claim to Curative. If anything is owed (a home-visit travel fee, for example), it is billed to the card on file.

Generally no out-of-pocket cost

As long as lactation is covered under your specific Curative plan and you have completed your baseline visit, lactation visits typically have no copay, coinsurance, or deductible applied.

Mom and baby billed separately

If mom and baby have different insurance plans, we submit a claim for each patient under the appropriate plan. Provide insurance information for both during intake.

Home visits covered, travel fee patient-billed

Home visits are covered under Curative the same way as office visits. The $70 home-visit travel fee is billed directly to your card on file and is not submitted to insurance.

Multi-state coverage, with our visits in Texas

Curative operates in select metros across Texas (Austin, Dallas, Houston, San Antonio), Florida (Miami, Orlando, Tampa), Georgia (Atlanta), Maryland, and the Washington DC area. If your Curative coverage is active, we can bill your lactation visit at our Houston or San Antonio offices regardless of where you live. Verification confirms your plan is active before your visit.

  • Chiropractic care, bodywork, and laser therapy are cash-pay services, not billed under your medical plan

Will I have any out-of-pocket costs for a lactation visit?

For most Curative members, no. As long as your plan includes lactation coverage and your baseline visit is complete, lactation visits typically come back $0 patient-responsibility on the claim. Verification will confirm this for your specific plan before your first visit.

Are home visits covered?

Yes. The lactation consultation portion of a home visit is billed to Curative just like an in-office visit. There is a separate $70 travel fee for home visits that is billed directly to your card on file and not submitted to insurance.

My employer just switched to Curative. What should I expect?

Curative is fairly straightforward to bill once benefits are verified. The most common surprise is the baseline-visit requirement: if you have not completed your annual baseline visit with Curative, your plan may not yet be in its $0-deductible state. That can affect how lactation claims process. Submit verification and we will check your specific status.

Do I need to wait for my insurance to be verified before booking my appointment?

No. You can book your appointment before your insurance is verified. As long as you submit your insurance information at least 1–2 business days (Monday through Friday) before your appointment, our team will have enough time to verify your coverage prior to your visit. If you are submitting the day before your scheduled visit, please submit by 3 PM so we have time to verify.

If we discover that your plan is not covered or has unusual restrictions, we will notify you by email and text message. If you decide to cancel your appointment due to lack of coverage, we will waive the cancellation fee as long as you request cancellation on the same day we notify you.
